Ability to have a survey or questionnaire for post event feedback

We would love to add a small form/questionnaire to our email sent to the organizer post-event, to get feedback on each event. We'd like to analyze the data regularly so we need some kind of structure, and not just text feedback, rather answer to questions.

We also have some additional questions (no pun intended) for you..
-In your current workflow, do you collect questions from guests at the time of contract signing or after the contract is signed, or both?
-If the questions are tied to the proposal, when should the guest no longer be able to edit/answer the questions?
-Do the event questions vary event by event?

I think it would be great to have a survey form that you can attached to an automated message once an event is marked completed

I added in an automatic email that's sent 2 days after the event with a link to a google form for feedback!
